JWCH looking for experienced Data Specialist with Case Management & Health Background to join our team and to help serve our rapidly grouping organization. This position will be located at our corporate office and be part of our Enhanced Care Management Department focusing primarily on our case management programs. Β This position requires knowledge of Excel, Access and other databases. Must have a good understanding of macro’s pivot charts/tables, queries, filters and data analysis in a health care setting. In addition, experience using web-based data portals, SFTP’s, writing and running EMR reports/queries.Β 

Position Purpose:
The Case Management Data Specialist will be responsible for the development, collecting, reporting and tracking encounters, quality outcome data and using health record NextGen, i2i /HEDIS measures and health plan reports. This position will also work with other ECM Administrative Support staff to support our Case Managements and Housing navigators located throughout Southern California. This may include processing new referrals, calling new patients and completing initial intakes as well as other supportive functions. Upon request will implement and maintain ECM/Behavioral Health/Substance Use Disorder/HIV data and recurring reports as well as dashboards that utilize the underlying data stores for internal and external use.Β 

Working with cross-functional teams, the Data Analyst will also assist other programs including Enhanced Care Management, Administration and Finance Department with their Data reports including Capitation and FFS Reconciliation reports, Monthly Dashboards, Provider Productivity, etc. thoroughly analyze and clarify report requirements, make recommendations, and educate when necessary to meet business needs. They will also be responsible for creating and maintaining internal controls and reference materials to promote data quality.

Principal Responsibilities:

  1. Collecting, reporting and tracking encounter and outcome data from NextGen, i2i and other health plan data systems for Enhanced Care Management, Behavioral Health HIV and SUD Finance and Administration.Β 
  2. Process new referrals, assign to case managers, call patients for initial intakes, process daily productivity reports, assist with staffing and capacity reports, update ECM Master list, as well as other duties assigned by Admin Supervisor or Director.Β 
  3. Β Mine data from primary and secondary sources, then reorganize said data in a format that can be easily read by organization and users
  4. Use statistical tools to interpret data sets, paying particular attention to trends and patterns that could be valuable for diagnostic and predictive analytics efforts.
  5. Demonstrate the significance of work in the context of local, national, and global trends that impact both their organization and industry.
  6. Prepare reports for Department Directors and executive leadership that effectively communicate trends, patterns, and predictions using relevant data.
  7. Collaborate with programmers, IT, and organizational leaders to identify opportunities for process improvements, recommend system modifications, and develop policies for data governance.
  8. Create appropriate documentation that allows stakeholders to understand the steps of the data analysis process and duplicate or replicate the analysis if necessary.
  9. Develop and maintain any necessary reference materials, such as data dictionaries, code books, and report specifications.
  10. Develop business intelligence dashboards.
  11. Use a design thinking approach to deliver data visualizations that are useful, clear, and insightful, given the intended audience(s)
  12. Perform other duties as assigned by supervisor.

Requirements:

  1. Bachelor’s degree in Computer Science, Management Information Systems, or a related field.
  2. At least 2 years’ experience working with data in the Health Care or Behavioral Health field.Β 
  3. Advanced proficiency in Excel, Access, and other database tools.
  4. Prefer prior experience in NextGen Electronic Health Record system or similar EMR.
  5. 100% on-site no remote
